Unlike other programs, Carewise Case Management offers members the opportunity to engage with a health coach who helps them learn to take an active role in managing their own health and fosters open communication with healthcare providers. A health coach provides the support that leads to self-accountability, self-confidence and a greater understanding of health risks and how to manage those risks.
The health coach works with members and their families to ensure the member’s treatment plan is consistent with best practices and is understood by everyone involved. The coach provides the information and support that the member and caregivers need to execute the treatment plan and weigh alternatives if necessary.
The coach helps members use their policy benefits effectively and confirms that only medically appropriate services are provided, at a competitive rate and at the most cost-effective level of care. Where needs exceed benefits, the coach helps the member identify and engage alternative resources.
