• Assessments. Early risk assessments identify individual needs and establish a baseline against which an expectant mother’s health can be measured. Program milestones include 16- and 28-week assessments, as well as a post-partum call.
• Personalized counseling. Carewise Maternity Management offers flexible contact schedules to meet each participant’s unique needs. The frequency of counseling calls conducted are based on risk level and participant preference. It also provides a variety of education materials and appropriate community referrals.
• High-risk programs. Maternity nurses apply standard criteria to identify high-risk cases. If an expectant mother is identified as high-risk, her nurse will develop a personalized care plan that addresses specific, high-risk diagnoses.
